Waiver Form
This form must be signed by all patrons of the Little Smoky Ski Area. Skiers/Snowboarders under the age of 18 years old must have their parent/guardian sign before accessing the slopes.
Instructions: Please read carefully.
1. An initial is required in the boxes at the top of the form for each individual skier named in the box at the bottom of the form. If the individual is under 18 years of age, the initial must be that of a parent or guardian.
2. A signature is required next to each individual skier named in the box at the bottom of the page. If the individual is under 18 years of age, the signature must be that of a parent or guardian.
3. Each signature requires a witness signature.

If you choose to rent equipment from the Little Smoky Ski Area, please complete and sign this form. Parent/Guardian signature required for skiers/snowboarders under 18 years of age.
